Birthday Wishes Please Fill out completely and email to Birthday Wishes Thank you for booking your event with Birthday Wishes. To ensure a successful event, please review the following policies that have been established regarding your party. This signed contract along with your deposit confirms the reserved party date and time requested for your event. Until signed contract, and your deposit has been received the reservation is not guaranteed and is subject to booking by another party. RESERVATIONS: A security deposit of $50.00 and a signed, approved contract are required to secure event dates and times. Events are treated as tentative until the contract and deposit are received. A deposit is stipulated in this contract and is based on this event. The deposit will be applied to the final invoice. CANCELATIONS: Once the deposit has been paid, events/parties can only be postponed. We gladly work with you to find an alternate date for your party based on availability. If you choose to cancel our service for any reason your deposit is non-refundable. FINAL PAYMENT: Final payment is due at the completion of your event. All charges will apply to a single form of payment. Other payment arrangements need to be approved by Birthday Wishes no less than one week before your event date. All checks must clear the bank prior to your event. MISCELLANEOUS FEES: Additional fees may also apply for distance traveled over 30 miles. LIABILITY: Birthday Wishes does not assume responsibility for personal property. Client agrees to indemnify and hold harmless Birthday Wishes/or any employee of Birthday Wishes for any unforeseen actions, accidents, injuries before, during or after the event/party. They also agree to provide a safe, clean, and smoke free environment/area for all parties’ activities. Client agrees to remain at the party at all times and will handle all discipline and behavior issues with party guests. DAMAGES: You will be held responsible for any damages to our property, if caused by a member of your party during your event due to gross misconduct or negligence of guests. PERFORMANCE: This agreement is contingent upon the ability of Birthday Wishes to complete same and is subject natural disasters, power outages, and any other cause whether enumerated herein or not. Customer agrees to the above terms and conditions and shall indemnify and hold Birthday Wishes harmless of any damage or loss arising from the scheduled event. All of this is agreed upon with the signing below. By signing below, I acknowledge that I have read, understand, and consent to the conditions of payment and conditions of service. (A copy of our FAQ’s and conditions of service has been included with this party agreement.)
